Insurance Policy

We are contracted with most major insurance carriers and are constantly reviewing new plans for participation. Please verify that we are a participating provider in your insurance plan by contacting your insurance carrier. Please inform the front office staff any time your insurance plan changes so we may update our files. If your insurance plan is not accepted at our office, call the office to speak to the receptionist about our self-pay pricing options. If you have an HMO plan, please make sure that Dr. Steven A. Swaldi is listed as your PCP on your insurance card.
